چکیده
M e t h o d s Data sources: PubMed and the Cochrane Library. Study selection and assessment: Randomized controlled trials (RCTs) that compared β-blockers as first-line treatment with placebo, no treatment, or other antihypertensive drugs in patients with primary hypertension, and evaluated all-cause mortality, cardiovascular morbidity, or both. Subgroup analyses were conducted for nonatenolol β-blockers, mixed β-blockers or diuretics, and atenolol. Outcomes: All-cause mortality, stroke, and MI.

[Should beta-blockers remain first choice in the treatment of primary hypertension?].
BACKGROUND Beta blockers have been used widely in the treatment of hypertension and are recommended as first-line drugs in hypertension guidelines. However, a preliminary analysis has shown that atenolol is not very effective in hypertension. We aim to substantially enlarge the data on atenolol and analyse the effect of different beta blockers. METHODS The Cochrane Library and PubMed were sea...
متن کاملShould beta-blockers still be used as initial antihypertensive agents in uncomplicated hypertension?
Beta-blockers have long being used as first-line therapy for hypertension as their use had resulted in a reduction in cardiovascular morbidity and mortality in controlled clinical trials. A recent meta-analysis comparing beta-blockers to all other anti-hypertensive drugs taken together has found that stroke reduction was sub-optimal. Specifically, atenolol was associated with a 26% higher risk ...
متن کاملEffectiveness and safety of beta blockers in the management of hypertension in older adults: a systematic review to help reduce inappropriate prescribing
BACKGROUND The benefit from a blood pressure lowering therapy with beta blockers may not outweigh its risks, especially in older populations. The aim of this study was to look for evidence on risks and benefits of beta blockers in older adults and to use this evidence to develop recommendations for the electronic decision support tool of the PRIMA-eDS project. METHODS Systematic review of the...
متن کاملDoes atenolol differ from other β-adrenergic blockers?
BACKGROUND A recent meta-analysis of drug effects in patients with hypertension claims that all beta-adrenergic blockers are equally effective but less so than other antihypertensive drugs. Published comparisons of the beta-adrenergic blocker atenolol and non-atenolol beta-adrenergic blockers indicate different effects on death rates, arrhythmias, peripheral vascular resistance and prognosis po...
متن کاملPrevention of risk factors: beta-blockade and hypertension
Some national guidelines on hypertension have demoted beta-blockers from a first-choice to a fourth-choice treatment. In contrast, the 2007 guidelines of the European Society of Hypertension/European Society of Cardiology (ESH/ESC) retain them among the drug classes used to initiate and maintain antihypertensive treatment, together with diuretics, angiotensin-converting enzyme (ACE)-inhibitors,...
